Family Care Partners welcomes three new “Care Coordinators”:
JoAnn Grover, Danica Mitten, and Dolores Wojnowicz.
The goal of our care coordinators is to provide our Medicare patients with a tightly integrated healthcare experience from our primary care provider and all points in-between, such as hospitals, specialists, diagnostics, and rehabilitation.
Our Care Coordinators are Registered Nurses (RN’s) fluent in case-management, and ensure that the appropriate resources and protocols are applied in an efficient and cost-effective manner, including the most current and maintained medical records.
Dolores Wojnowicz, RN, BA, CMC, has 35 years of nursing and case management experience, and earned her Bachelor’s in Education from Plattsburgh State University and her RN degree from Upstate Medical Center in New York, and maintains a Case Management Certification.
Danica Mitten, RN, is an Army veteran, and earned her degree in Nursing from Northern Virginia Community College and an Associate of General Studies degree from Central Texas College.
JoAnn Grover, RN, BSN, CCM, CDMS, has a broad range of healthcare experience spanning 35 years. Ms. Gover earned her Bachelor of Science in Nursing, from the University of Tennessee in Knoxville, and participates in the Case Management Society of America, and has helped develop rehabilitation programs with CSX Railroad.
