This year the FCP Spirit of Care will be contributing the funds raised to the First Coast Cancer Foundation. This foundation will benefit men in Jacksonville that have been diagnosed with prostate cancer. This past week we celebrated National Medical Laboratory Professionals Week. We here at Family Care Partners have the best phlebotomists and lab technicians around!! I hope that everyone who came by showed their appreciation for all of their hard work and dedication to providing the best care possible for all of our wonderful patients. A big thank you to all who brought gifts and goodies to the lab!! Greatness is achieved when we work hard and do our best, and our lab staff definately contributes to the GREATNESS of Family Care Partners!! Keep up all the good work!!

After 12 years of practicing Family Medicine Dr. Jennifer Glassman has been awarded the “Degree of Fellow” from the American Academy of Family Physicians. According to the AAFP, the Degree of Fellow recognizes AAFP members who have distinguished themselves among their colleagues, as well as their communities, by their service to Family Medicine, by the advancement of health care of the American People and by their professional development through medical education and research.
“It has been my personal and professional goal for the past 10 years and I will be awarded the Degree at the Fellow Convocation during the Annual Scientific Assembly of the AAFP in Denver, Colorado in October, 2010.”

WALK TO DEFEAT ALS
Family Care Partners is joining the fight against Lou Gehrig’s Disease.
FCP clinicians and employees will Walk to Defeat ALS on Saturday April 24th, 10:00am at the Seven Bridges Grille in Jacksonville.
Amyotrophic Lateral Sclerosis (ALS), often referred as Lou Gehrig’s Disease, is a progressive, fatal neuromuscular disease that slowly robs the body of its ability to walk, speak, swallow and breathe. The life expectancy of a person with ALS averages 2 – 5 years from the time of diagnosis. ALS can strike anyone at anytime. This disease knows no racial, ethnic or socioeconomic boundaries.
Every gift makes a difference! Help us support ALS and the fight against Lou Gehrig’s Disease!
Money raised for this charity helps those afflicted with this devastating disease. Donations help the ALS Association provide funding for research, education, resource groups, medical equipment and communication devise loans to patients, and more!

Walk to Cure Diabetes!
The Juvenile Diabetes Research Foundation International (JDRF) is the worldwide leader in funding research to cure type 1 diabetes, an autoimmune disease that strikes children and adults suddenly and lasts a lifetime.
In 2008, JDRF provided more than $156 million to diabetes research, and JDRF is responsible for more than $1.3 billion in direct funding since it was founded. 85 percent of any money raised or donated goes directly to research and support for diabetes.
Diabetes is a chronic, debilitating disease affecting every organ system. There are two major types of diabetes: type 1 and type 2. Type 1 diabetes is an autoimmune disease in which a person’s pancreas stops producing insulin, a hormone that enables people to get energy from food. Type 2 diabetes is a metabolic disorder in which a person’s body still produces insulin but is unable to use it effectively. Some warning signs for Type 1 diabetes include extreme thirst, frequent urination, increased appetite, sudden weight loss, fruity or sweet smelling breath.

Family Care Partners, a leader in family practice medicine for Jacksonville, wants to let everyone in on what’s coming up in March!! This March we are raising money for the “Go Blue for Colon Cancer” campaign. We will be raffling, collecting, and employees will be participating in a dress down in blue day which they will be donating $5 to participate in. That will be held on March 5th nationwide. In the tradition of the New Year, many set resolutions to make themselves better. Family Care Partners of Jacksonville would like to help you set a realistic New Year’s Resolution for 2010!
When setting a resolution, make it simple so you will stick to it! Think about a resolution as a goal or objective for the new year. Be realistic in setting your resolution.
Describe your resolution in specific terms, for example: I will lose weight by going to the gym daily and eating smaller portions.
Break down large goals into small ones like changing lose weight into resolve to go to the gym, improve eating habits.
Find alternatives to a behavior you want to change. If you smoke to relax, find something else to help you relax.
Aim for the goals that are truly important to you, not what you think you ought to do, or what others expect of you!

Family Care Partners of Jacksonville wants to wish everyone a very Merry Christmas. Christmastime is a time for family, and not only does it feel like a family here at FCP, we want to make our patients feel like we are here for their families. With our pediatrics department, and adult medicine, also lab and x-ray, all trying to make your experience as comfortable and convienient as possible. Hopefully making it so you can get in and out quickly and get back to your own families especially during this time of year. FCP would also like to recognize all the caring people that took Angels from the Salvation Army Angel Tree, and opened their hearts and wallets to a bunch of children who otherwise would not have gotten anything. Again, wishing you a joyous holiday season, hold your loved ones close to your heart, and don’t forget to tell them they are there. Get into the spirit of giving this year and reach out into your community and help someone who needs it. And, God bless us, EVERYONE!

Family Care Partners has been notified that our practice has been selected for inclusion in the next volume of Quality Profiles: The Leadership Series which will be published in December 2009. The Leadership Series this year focused on advancing population health through the use of health information technology (HIT). FCP responded to the call for Submission of Best Practices and provided NCQA and Pfizer with information related to the use of our EMR to improve the diabetic outcomes of our patients.
Quality Profiles: The Leadership Series article ” Utilization of HIT to Standardize and Improve the Quality of Care for Patients With Diabetes Mellitus” describes how FCP improves the outcomes of our diabetic patients though our use of our robust EMR. The ability of our EMR to provide our clinicians with health decisions at the time of care and the ability to run reports allowed us achieve positive patient outcomes. Once we receive copies of the volume, we will share it with the staff.
